Physio Penrith Low Back Pain With Stiffness Treatment

Treatment of Low Back Pain with Stiffness at Sydney Muscle & Joint Physio Penrith

What is low back pain with stiffness?

Low back pain and stiffness is a very common presentation and it is expected that 85% of us will experience back pain in our lives, often in repeat episodes. More often than not low back pain with stiffness is due to restricted lumbar joints, but you may experience associated muscle tightness. Restricted lumbar range of motion ensues and this is in turn contributes to joint stiffness. At Sydney Muscle & Joint Physio Penrith we use research supported interventions in the management of low back pain with stiffness. These include both passive and active techniques that are proven to work and include spinal manipulation (where needed), joint mobilisation, McKenzie Method repeated movements, soft tissue work, nerve stretching techniques and the very latest in trunk and back strengthening and endurance exercises.

What are the different categories of low back pain with stiffness?

Acute low back pain is defined as back, buttock, or thigh pain with a duration of 1 month or less. Common symptoms include one sided back pain and the onset of symptoms is often linked to a recent unguarded/awkward movement or position. Lumbar range of motion limitations show restricted lower thoracic and lumbar segmental motion, low back and low back–related lower extremity symptoms are reproduced with provocation of the involved lower thoracic, lumbar, or sacroiliac segments. At Sydney Muscle & Joint Physio Penrith of physiotherapists treatment acute low back pain with stiffness with some manual therapy procedures such as thrust manipulation and other non-thrust mobilisation techniques to diminish pain and improve segmental spinal or lumbo-pelvic motion. Further therapeutic exercises such as McKenzie method directional preference movements are used to improve or maintain spinal mobility. Our physio's support the patient maintaining an active lifestyle.

Subacute low back pain has a presentation of 4-12 weeks, most commonly is also unilateral or one sided pain in the low back, buttock, or thigh with increased reporting of the sensation of back stiffness. Physiotherapists will reproduce symptoms with end-range spinal motions- meaning there is more range before provocation of pain. Symptoms reproduced with provocation of the involved lower thoracic, lumbar, or sacroiliac segments. At Sydney Muscle & Joint Physio Penrith sub-acute low back pain will present with 1 or more of the following: restricted thoracic range of motion and associated segmental mobility; restricted lumbar range of motion and associated segmental mobility; restricted hip range of motion and associated accessory mobility. Our physiotherapists in Penrith apply research supported interventions in the management of sub-acute low back pain and these include manual therapy procedures such as spinal manipulation or joint mobilisations to improve segmental spinal, lumbopelvic, and hip mobility. Further therapeutic exercises such as McKenzie method repeated movements are used to improve or maintain spinal and hip mobility. Research further supports the use of preventive measures for recurring low back pain episodes through the use of exercise that address any co-existing coordination impairments, strength deficits, and/or endurance deficits. Our physio's support the patient maintaining an active lifestyle.

Best practice interventions for the treatment of low back pain with stiffness?

Manual therapy: our physio's utilise spinal manipulative procedures to reduce pain and disability in patients with mobility deficits and acute low back and back-related buttock or thigh pain. Spinal manipulative and non-thrust joint mobilisation procedures can also be used to improve spine and hip mobility and reduce pain and disability in patients with subacute and chronic low back and back-related lower extremity pain.

McKenzie Method: incorporates centralisation and directional preference exercises and procedures in the form of repeated movements and exercises to promote centralisation of symptoms in patients with low back pain with related (referred) lower extremity pain. At Sydney Muscle & Joint Physio Penrith our physiotherapists and chiropractors use repeated exercises in a specific direction determined by treatment response to improve mobility and reduce symptoms in patients with acute, subacute, or chronic low back pain.

Trunk coordination, strengthening and endurance exercise: at Sydney Muscle & Joint Physio Penrith our physiotherapists utilise the very latest in trunk coordination, strengthening, and endurance exercises to reduce low back pain and disability in patients with all types of low back pain. These include anti-movement strengthening exercises within pain free ranges of motion and can be performed even during an episode of back pain as they are safe and do not irritate back pain conditions.

Patient education and advice: at Sydney Muscle & Joint Physio Penrith our strategies, education and advice for patients with low back pain is the promotion of the understanding of the inherent strength, robustness and resilience of the human spine, further our physiotherapists emphasise the overall favorable prognosis of low back pain, we decrease fear and catastrophising, encourage the early resumption of normal or vocational activities, even when still experiencing pain, and we focus on the importance of improvement in activity levels, not just pain relief.
