NDIS Needs Analysis - Information gathering
After initial contact from yourself, our practice manager will schedule a conversation to gather all the information we need to help with the shared decision making process. This information is then passed onto our NDIS approved physiotherapist and exercise physiologist at which time they will schedule another phone call with you to discuss clinical objectives, your goals, previous treatments and other practitioners involved in your treatment and they will discuss the service agreement and schedule your initial consultation.
After we have discussed your needs, funding available and the most appropriate schedule for you to reach your goals - we put together a service agreement to ensure full transparency in our services.
During our initial consultation our physiotherapists and exercise physiologists will perform a specific assessment and use specific outcome measures to collect data on your current capacity. This ensures our treatments target any of the impairments we believe will help you achieve your goals.
Our ongoing consultations are at discussed durations and frequency that is best for you and your timetable. This consistency ensures that we have the ability to help you succeed and achieve your goals within the allocated funding that you wish to use for our services.
